Your responses to the following questions will form the foundation for your professional development plan so please answer them as candidly as possible. The scoring system is designed to identify your strengths and areas for growth and are not in comparison to other people. To this extent the scores are only meaningful in relation to your other scores.

There are three parts to this assessment. In each part, read the questions and respond in the box provided. Each part will require about 30-60 minutes to complete. Do not spend more than 3-5 minutes on each item.

Upon completion of each part, be sure to print your responses for your records.
